My fasting numbers are good (85–90) but post-meal readings are consistently hitting 140–155 despite eating carefully. My target is under 120.
Post-meal spikes despite a controlled diet suggest your pancreas cannot produce enough insulin for the meal volume — this is the nature of GD and it's not your fault. A few things to discuss with your doctor: (1) Is your target under 120 at 1 hour or 2 hours? Standards vary — 1-hour under 140 and 2-hour under 120 is one common guideline. (2) Your doctor may recommend starting insulin for meal coverage — this is not failure, it's just your pancreas needing support. (3) The 15-minute post-meal walk can bring readings down 15–25 points. If diet and exercise consistently miss targets, insulin is the appropriate next step and is safest for baby.
My readings were similar and my OB started me on very low-dose insulin at dinner. Within a week all my numbers were in range and I felt less stressed because I wasn't obsessively restricting food. Insulin in GD is not a failure — it's a physiological response to your placenta producing insulin-blocking hormones. Many thin, fit women with no risk factors develop GD for exactly this hormonal reason.
